(If playing in a June 24-26 tournament, an entry form must be emailed/faxed to us by 10:00 am on Monday June 27th.
  2. Having your at-large registration and payment into the MBT office and then being awarded an at-large berth. Once you qualify, send the following information to MBT by the required dates! 1) Email the Entry Form to MBT that was in your MBT Berth Envelope 2) Have the Roster-Waiver Form Completed and signed (Online Roster Form) 3) Submit the proper Entry Fee 4) PLEASE MAKE COPIES OF ALL FORMS AND KEEP WITH YOU!
